Found what looks like a match on ebay. A guy is selling several for $99 each: http://www.ebay.com/itm/ 251382361230 ¹⁾ .

“Bicron NaI scintillation detector (model: 1.12×1.12M3/1.12L). Meter is not for sale. Background measures around 8k cpm at 650 volts. Also can be used for gamma spectroscopy (see picture). This spectrum is of rich thorite using a gamma spectacular set at 650 volts. Also shown is it responding to samples of uraninite and a uranium ore core sample. It has a bnc connector coming out of the detector, so you will need an adapter (not included) like the one pictured to use it on other type connections (most Ludlums). Dimensions are 9.5” x 1.375” x 1.375“

Mine appears to be from a different production run from above but appears to be fit and form identical.
