Found in a thermocoupler-signal-conditioner hybrid module from a telemetry system.
This is a quad SPST analog switch. I can't tell from the die and databooks alone whether this is an ADG211A/212A, ADG221/222, or something newer such as the ADG411/412, ADG441/442, or ADG444/445.
