From eBay seller allenandallen.  I have two of these.  One has written on bottom "41-661-12" and the other has "41-661-5".

5 small test patterns in + sign formation spread throughout the wafer.  One larger test section (didn't photograph this).  Looks to be RAM or a regular structure of some sort
